WaSH<\/b><\/a><\/p>\n<p><b>Context:<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">The recent article \u201cCan sanitation reduce stunting?\u201d (<\/span><i><span style=\"font-weight: 400;\">The Hindu<\/span><\/i><span style=\"font-weight: 400;\">, February 15) touched upon many dimensions and possible reasons to explain why <\/span><b>Water, Sanitation and Hygiene (WaSH) trials in countries like Kenya and Bangladesh ended, disappointingly, with no significant reduction in stunting among children.<\/b><\/li>\n<\/ul>\n<p><b>More in news:<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">When the effect of poor sanitation is obviously passing on from one generation to the other, <\/span><b>it might take at least a generation to adopt WaSH interventions before their outcomes can be seen.<\/b><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Therefore, short-term trials like the ones in <\/span><b>Kenya and Bangladesh are bound to show little or no effect.<\/b><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">In addition, in India, where the baseline, unlike in those countries, is so large (over 50% of open defecation against 1% in Bangladesh) <\/span><b>even small improvements can demonstrate significant and palpable changes.<\/b><\/li>\n<\/ul>\n<p><b>Stunning:<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Stunting is driven by <\/span><b>multiple factors, one of which is inflammation.<\/b><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Inflammation is <\/span><b>normal biological responses of body tissues to stimuli such as disease-causing bacteria (pathogens), but ironically repeated exposure to high doses of bacteria that are not linked with diseases or diarrhoea also cause inflammation.<\/b><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Inflammation <\/span><b>down regulates growth factors, and thus impairs normal growth in children.<\/b><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Mothers with inflammation in the gestation tissues had <\/span><b>smaller babies in our study.<\/b><\/li>\n<\/ul>\n<p><b>India needs to learn from Bangladesh:<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">It is indeed true that mere building of toilets <\/span><b>cannot prompt people to use them as there are a lot of social, cultural and behavioural aspects attached to it.<\/b><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">What India needs to learn from Bangladesh is <\/span><b>how they have managed to bring down open defecation to less than 1% by 2016, from a whopping 42%, in a little over a decade.<\/b><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">A huge chunk of public and charity money was <\/span><b>spent on building toilets, and campaign volunteers slogged to change public attitudes and habits.<\/b><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Children were used literally as <\/span><b>whistle-blowers and agents of change while door-to-door campaigns were carried out.<\/b><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">It was done in a <\/span><b>dogged campaign in mission mode supported by 25% of the country\u2019s overall development budget.<\/b><\/li>\n<\/ul>\n<p><b>Way ahead:<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">The very fact that over half (about 52%) of rural India <\/span><b>still defecates in the open is still a reason why it may be too early to quash or discount \u00a0Swachh Bharat Abhiyan (SBA).<\/b><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">The importance SBA which accords to cleanliness, hygiene and sanitation <\/span><b>can go a long way in India\u2019s fight against not only stunting (low height for age)
