It is a significant step towards <span style=\"color: #ff0000;\">universalising healthcare in the country<\/span>. The Rs 5 lakh annual health coverage will <span style=\"color: #ff0000;\">benefit about six crore people<\/span>, which translates to roughly 5 per cent of the country\u2019s current population.<\/p>\n<p>In this article, we will look into the status of elderly in India, the expansion of Ayushman Bharat Scheme to include elderlies, the significance of the inclusion. What are the Health care concerns of senior citizens in India?<\/strong><\/span><\/h2>\n<p>The number of elderlies (persons above 60 years) in India is set to increase from <span style=\"color: #ff0000;\">100 million<\/span> in 2011 to <span style=\"color: #ff0000;\">230 million<\/span> in 2036. By 2050, the elderly population is expected to constitute nearly <span style=\"color: #ff0000;\">one-fifth of the total population<\/span>.<\/p>\n<p><strong>Increase in the old age-dependence ratio-<\/strong><br \/>\nThe old age-dependence ratio denotes the <span style=\"color: #ff0000;\">number of persons aged 60-plus per 100 persons<\/span> in the age group of 15-59 years.<\/p>\n<p>According to the Ministry of Statistics and Programme Implementation\u2019s (MOSPI) \u2018<span style=\"color: #ff0000;\">Elderly in India 2021<\/span>\u2018\u00a0report, the old-age dependency ratio is increasing in India. The old age-dependence ratio has increased from\u00a0<span style=\"color: #ff0000;\">10.9% in 1961<\/span>\u00a0to\u00a0<span style=\"color: #ff0000;\">14.2% in 2011<\/span>\u00a0and is projected to increase to\u00a0<span style=\"color: #ff0000;\">15.7% in 2021\u00a0<\/span>and\u00a0<span style=\"color: #ff0000;\">20.1% in 2031<\/span>\u00a0respectively.<\/p>\n<figure id=\"attachment_310110\" aria-describedby=\"caption-attachment-310110\" style=\"width: 500px\" class=\"wp-caption alignnone\"><img data-recalc-dims=\"1\" loading=\"lazy\" decoding=\"async\" class=\"wp-image-310110\" src=\"https:\/\/i0.wp.com\/forumias.com\/blog\/wp-content\/uploads\/2024\/09\/Health-Care-for-Older-Persons.png?resize=500%2C417&#038;ssl=1\" alt=\"Health Care for Older Persons\" width=\"500\" height=\"417\" \/><figcaption id=\"caption-attachment-310110\" class=\"wp-caption-text\">Source- MoSPI<\/figcaption><\/figure>\n<p><strong>Health care Concerns of Senior Citizens<\/strong><\/p>\n<p><strong>1. Rise in age-related chronic illness-\u00a0<\/strong>According to the Longitudinal Ageing Study of India (LASI) in 2021, <span style=\"color: #ff0000;\">One in five elderly persons in India has mental health issues<\/span> and around 75 per cent of them suffer from a chronic disease.<\/p>\n<p><strong>2. Increasing need for geriatric care-<\/strong> Increased health-related expenses for the treatment of diseases like <span style=\"color: #ff0000;\">Non-Communicable diseases<\/span>, <span style=\"color: #ff0000;\">cataract<\/span>, <span style=\"color: #ff0000;\">hearing loss<\/span> etc. creates financial problem for the elderly population.<\/p>\n<h2><span style=\"color: #000000;\"><strong><a id=\"toc2\"><\/a>What are the specifications of the expanded Health Cover for Senior Citizens?<\/strong><\/span><\/h2>\n<p><strong>AB PM-JAY-<\/strong> AB PM-JAY is already the <span style=\"color: #ff0000;\">world\u2019s largest public health insurance scheme<\/span>. It provides <span style=\"color: #ff0000;\">free cover upto Rs 5 lakh annually<\/span> to all members of eligible families, which are estimated to comprise the bottom 40% of the population economically, irrespective of age.<\/p>\n<p><strong>Health Cover Extension under AB PM-JAY-<\/strong> The Health cover for Senior Citizens has been expanded under the Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (AB-PMJAY). The expanded scheme will <span style=\"color: #ff0000;\">cover almost 6 crore individuals<\/span> (from 4.5 crore families), of whom<span style=\"color: #ff0000;\"> 1.78 crore are already covered by the scheme<\/span>.<\/p>\n<table style=\"width: 100%; border-collapse: collapse; border-style: solid; background-color: #fffceb;\">\n<tbody>\n<tr>\n<td style=\"width: 16.6667%;\"><strong>Beneficiaries<\/strong><\/td>\n<td style=\"width: 83.3333%;\"><strong>a.<\/strong> Anyone who has turned 70 as per their Aadhaar <span style=\"color: #ff0000;\">can register for the new health cover<\/span>.<br \/>\n<strong>b.<\/strong> Everyone of age 70 or more will be <span style=\"color: #ff0000;\">entitled to a cover of Rs 5 lakh annually<\/span>, <span style=\"color: #ff0000;\">shared within the family<\/span>. This means if there are two elderly beneficiaries in the household, the cover will be split among them.<br \/>\n<strong>c.<\/strong> Elderly members (age 70 and more) of families that are already covered in accordance with their economic status will get a<span style=\"color: #ff0000;\"> top-up cover of Rs 5 lakh<\/span>&#8211; to be used only for the elderly. These elderly beneficiaries will have to re-register to receive the top-up cover.<\/td>\n<\/tr>\n<tr>\n<td style=\"width: 16.6667%;\"><strong>Registration<\/strong><\/td>\n<td style=\"width: 83.3333%;\"><strong>a.<\/strong> Beneficiaries <span style=\"color: #ff0000;\">can register either through the government\u2019s Ayushman App<\/span> or at the <span style=\"color: #ff0000;\">Ayushman Bharat counter at any government health facility<\/span>.<br \/>\n<strong>b.<\/strong> Beneficiaries will be able to utilise their policy <span style=\"color: #ff0000;\">as soon as they complete their eKYC<\/span>. The portal will also be <span style=\"color: #ff0000;\">face-authorisation-enabled<\/span>, to ensure that even elderly persons whose fingerprints are difficult to read or have changed over time can avail the benefits.<\/td>\n<\/tr>\n<tr>\n<td style=\"width: 16.6667%;\"><strong>Exclusions and Inclusions<\/strong><\/td>\n<td style=\"width: 83.3333%;\"><strong>a.<\/strong> <strong>Option for already covered people-<\/strong> 80 lakh people are separately covered under various government health schemes- the <span style=\"color: #ff0000;\">Central Government Health Scheme<\/span> (CGHS), <span style=\"color: #ff0000;\">Ex-Servicemen Contributory Health Scheme<\/span>, or cover provided to family members of defence and railway personnel. Those who are covered under these government health schemes will have the <span style=\"color: #ff0000;\">option of choosing either Ayushman Bharat<\/span> or <span style=\"color: #ff0000;\">continuing with their existing coverage<\/span>.<br \/>\n<strong>b. People with ESIC coverage to be included-<\/strong> People who are covered under the Employees State Insurance Corporation (ESIC), however, <span style=\"color: #ff0000;\">will be eligible to have both their existing cover<\/span> and <span style=\"color: #ff0000;\">Ayushman Bharat<\/span>. This is because premiums for ESIC are paid by the insured and their employer, not the government.<br \/>\n<strong>c. People with private insurance-<\/strong> People who have their own privately purchased insurance cover will be <span style=\"color: #ff0000;\">eligible to have the Ayushman Bharat cover as well<\/span>.<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<h2><span style=\"color: #000000;\"><strong><a id=\"toc3\"><\/a>What is the Significance of the Expanded Health Cover for Senior Citizens?<\/strong><\/span><\/h2>\n<p><strong>1. Expansion of Health Care ambit for Senior Citizens-<\/strong> According to the India Ageing Report 2023 <span style=\"color: #ff0000;\">barely 20% of India\u2019s population above the age of 60<\/span> is currently <span style=\"color: #ff0000;\">covered by any government, employer-provided, or personal insurance scheme<\/span>. Extension of the ambit of the world\u2019s publicly-funded health assurance scheme is a crucial step towards ensuring a more fulfilling life for the senior citizens in India.<\/p>\n<p><strong>2. Increasing elderly population in India-<\/strong> According to the government\u2019s <span style=\"color: #ff0000;\">Longitudinal Ageing Study in India<\/span> (LASI), the percentage of elderly population over the age of 60 years is set to increase to <span style=\"color: #ff0000;\">19.5% by 2050<\/span> from <span style=\"color: #ff0000;\">8.6% in 2011<\/span>. In terms of numbers, Indians over the age of 60 are likely to be 319 million in 2050, which will be three times from the 103 million in 2011. This will increase the geriatric health care needs in India.<\/p>\n<p><strong>3. Increase in hospitalisation rates among elderly-<\/strong> The <span style=\"color: #ff0000;\">rate of admission<\/span> among the elderly already covered under the AB PM-JAY scheme <span style=\"color: #ff0000;\">has been more than 7%<\/span>, which is<span style=\"color: #ff0000;\"> double<\/span> the <span style=\"color: #ff0000;\">3-4% admission rate for the younger people<\/span> covered under the scheme.<\/p>\n<p><strong>4. Gender Justice by catering to elderly women-<\/strong> According to a survey by NITI Aayog, women constitute the larger share of the elderly population, with <span style=\"color: #ff0000;\">58% of the elderly being women<\/span>, and <span style=\"color: #ff0000;\">54% among them being widows<\/span>. The extension of health cover will <span style=\"color: #ff0000;\">provide immense help to women<\/span>, and <span style=\"color: #ff0000;\">especially widowed women<\/span>.<\/p>\n<p><strong>5. Reduction of the out-of-pocket health care expenditure-<\/strong> This will help in reducing the out-of-pocket expenditure (OOPE) for senior citizens, as the <span style=\"color: #ff0000;\">OOPE for health care in India is among the highest in the world<\/span>.<\/p>\n<h2><span style=\"color: #000000;\"><strong><a id=\"toc4\"><\/a>What are the challenges that remain despite the expansion of health coverage?<\/strong><\/span><\/h2>\n<p><strong>1. Low penetration of AB PM-JAY-<\/strong> There has been a lower penetration of PM-JAY into smaller cities and towns in most states, since its launch in 2018. This <span style=\"color: #ff0000;\">lower penetration will create a challenge in achieving universalisation of health care<\/span> needs of the elderly.<\/p>\n<p><strong>2. Absence of coverage of outpatient care-<\/strong> Most of the health-care expenditure by the elderly is through <span style=\"color: #ff0000;\">outpatient care<\/span> (<span style=\"color: #ff0000;\">40%-80%<\/span>). The <span style=\"color: #ff0000;\">absence of coverage for outpatient care<\/span>, <span style=\"color: #ff0000;\">diagnostics<\/span>, and <span style=\"color: #ff0000;\">medicines<\/span> for the elderly is particularly concerning as chronic diseases have increased sharply in India in the last few decades.<\/p>\n<p><strong>3. Limitation of the scheme to secondary and tertiary care hospitalisation-<\/strong> The expanded health assurance scheme remains limited to secondary and tertiary care hospitalisation. The<span style=\"color: #ff0000;\"> primary and secondary health care<\/span> in the public sector <span style=\"color: #ff0000;\">remains largely inadequate<span style=\"color: #333333;\"> and<\/span> ill-equipped<\/span> to meet the demands of the senior citizens.<\/p>\n<p><strong>4. Functional challenges with AB PM-JAY-<\/strong> There are serious functioning challenges in the implementation of AB PM-JAY, which are explained below-<br \/>\n<strong>a. Misinformation-<\/strong> There are numerous reports showing that doctors indulge in spreading misinformation to already stressed families of the grievously ill, that <span style=\"color: #ff0000;\">availing benefits under AB-PMJAY could take months<\/span>.<br \/>\n<strong>b. Overburdened Staff-<\/strong> There are reports regarding the overburdened staff at <span style=\"color: #ff0000;\">government institutions not wanting to take on the additional task of filling up documents related to the insurance scheme<\/span>.<br \/>\n<strong>c. Limited role of Arogyamitras-<\/strong> The role of Arogyamitras is limited to registering claims under the scheme. Their role has been limited in counselling relatives of patients.<\/p>\n<p><strong>5. Failure of insurance model over primary health care improvement- <\/strong>The reliance of U.S., on <span style=\"color: #ff0000;\">insurance-based schemes has led to the surging costs of health care<\/span>. While Thailand on the other hand has focused on progressively strengthening its primary health-care system and attaining universal health coverage.<\/p>\n<h2><span style=\"color: #000000;\"><strong><a id=\"toc5\"><\/a>What Should be the Way Forward?<\/strong><\/span><\/h2>\n<p><strong>1. Improvement in AB PM-JAY&#8217;s functioning-<\/strong> The Arogya mitras should be <span style=\"color: #ff0000;\">empowered to handhold patients<\/span> in cases of claim settlement and ironing out of glitches.<\/p>\n<p><strong>2. Expansion of Health care infrastructure-<\/strong> According to NITI Aayog&#8217;s report, the increased nuclearisation of families is making the senior citizens more vulnerable to health care needs. Policymakers should aim to improve the number of health care centres and to <span style=\"color: #ff0000;\">narrow down the doctor-patient asymmetry<\/span>.<\/p>\n<p><strong>3. Increasing the robustness of primary and secondary public health care-<\/strong> Robust primary and secondary public health care <span style=\"color: #ff0000;\">will sharply cut the load on tertiary health care<\/span> and <span style=\"color: #ff0000;\">the need for curative care<\/span>. A reduced curative care load at tertiary hospitals will help in the success of PM-JAY scheme.<\/p>\n<p><strong>4.
