Group health covers <strong>25.59 crore lives<\/strong> with \u20b9<strong>55,666 crore<\/strong> (52%) and government schemes cover <strong>26.11 crore lives<\/strong> with just \u20b9<strong>10,513 crore<\/strong>(9.8%). <strong>Average premiums<\/strong> reflect this divide \u2014 \u20b9<strong>7,437<\/strong> (individual), \u20b9<strong>2,175<\/strong> (group), and \u20b9<strong>402<\/strong> (government).<\/li>\n<li><strong> High Loss Ratios Across Segments:<\/strong> The overall <strong>incurred loss ratio<\/strong> was <strong>88.15%<\/strong>, with <strong>group health at 94%<\/strong>, <strong>government schemes at 115.28%<\/strong>, and <strong>individual at 75%<\/strong>. These indicate <strong>sustainability challenges<\/strong>, especially for public-funded programs.<\/li>\n<li><strong>Insurer-Wise Claims Experience:<\/strong> <strong>Public sector insurers<\/strong> had a <strong>103%<\/strong> incurred claims ratio, while <strong>private<\/strong> stood at <strong>89%<\/strong>, and <strong>standalone health insurers<\/strong> at <strong>65%<\/strong>, reflecting differences in <strong>risk strategy<\/strong> and <strong>pricing models<\/strong>.<\/li>\n<\/ol>\n<h2><strong>Geographic and Provider Concentration<\/strong><\/h2>\n<ol>\n<li><strong>Urban-Centric Premium Collection<\/strong><strong>: <\/strong>Nearly 64% of premiums come from just five regions: Maharashtra (29.5%), Karnataka (11%), Tamil Nadu (10%), Gujarat (7%), and Delhi (6.5%), showing limited geographical spread.<\/li>\n<li><strong> Private Sector Dominance in Care Delivery:<\/strong> Private healthcare accounts for <strong>65% of hospitalisations<\/strong> in urban and <strong>54% in rural<\/strong> areas. It also delivers <strong>74% of outpatient<\/strong> care in cities and <strong>67% in villages<\/strong>, indicating <strong>dependency on private services<\/strong>.<\/li>\n<li><strong> High Costs for Serious Illnesses: Private care is significantly costlier \u2014 up to 6.8x higher for cardiac cases and 5.2x for respiratory illnesses. This worsens <\/strong><strong>financial stress<\/strong><strong> and raises <\/strong><strong>premium costs<\/strong><strong>.<\/strong><\/li>\n<\/ol>\n<h2><strong>Key Systemic Issues<\/strong><\/h2>\n<ol>\n<li><strong>Incomplete and Denied Claims:<\/strong> Only <strong>71.31% of claims<\/strong> (\u20b983,493 crore) were paid in FY24. <strong>22.22%<\/strong> (\u20b926,037 crore) were unpaid \u2014 with \u20b9<strong>15,100 crore disallowed<\/strong> under policy terms and \u20b9<strong>10,937 crore repudiated<\/strong>. \u20b9<strong>7,584 crore<\/strong> remained <strong>pending<\/strong>.<\/li>\n<li><strong>Need for Better Oversight: <\/strong>There are large variations in settlement practices among insurers. IRDAI review, policy standardisation, and Board-level monitoring are needed for transparency and fairness.<\/li>\n<li><strong>Provider Pricing Disparities:<\/strong> Treatment costs vary widely, even within the same <strong>hospital groups<\/strong> across cities. This complicates <strong>premium calculations<\/strong>and requires a <strong>uniform regulatory framework<\/strong>.<\/li>\n<\/ol>\n<h2><strong>Reform Recommendations and Structural Changes<\/strong><\/h2>\n<ol>\n<li><strong> Promote Standard Policies: <\/strong>The <strong>Arogya Sanjeevani policy<\/strong>, with <strong>uniform terms<\/strong>, should be scaled up to <strong>minimise <\/strong><strong>dispute and <\/strong>bring <strong>pricing consistency<\/strong>.<\/li>\n<li><strong> Establish a Healthcare Regulator:<\/strong> A national regulator is needed to handle <strong>pricing transparency<\/strong>, <strong>medical inflation<\/strong>, and <strong>advanced technologies<\/strong> in health coverage.<\/li>\n<li><strong> Strengthen Tier-2\/3 Infrastructure:<\/strong> Public health expansion can be financed via <strong>health bonds<\/strong> issued by <strong>municipal authorities or hospitals<\/strong>, eligible for insurer investments.<\/li>\n<li><strong> GST Relief for Seniors:<\/strong> Reducing the <strong>18% GST<\/strong> on health insurance to <strong>zero for senior citizens<\/strong> will make coverage more affordable.<\/li>\n<li><strong>Process and Outreach Reforms:<\/strong> Insurers must adopt <strong>digital tools<\/strong>, improve <strong>claims efficiency<\/strong>, monitor <strong>fraud<\/strong>, and cut <strong>intermediation costs<\/strong> by promoting <strong>direct sales<\/strong>.<\/li>\n<\/ol>\n<h2><strong>Public Sector Impact and Future Strategy<\/strong><\/h2>\n<ol>\n<li><strong> Welfare Gains from PMJAY : <\/strong>The PMJAY scheme offers \u20b95 lakh coverage per family and has reduced out-of-pocket spending by \u20b91.25 lakh crore.
