While these initiatives expand access, they raise concerns about how far <strong>citizens can engage with and influence health governance<\/strong> at different levels.<\/p>\n<h2><strong>Evolving Nature of Health Governance<\/strong><\/h2>\n<ol>\n<li><strong> From Government-led to Multi-actor Systems:<\/strong> Health governance now includes <strong>civil society, professional associations, hospital bodies, and trade unions<\/strong>. It works through both formal and informal processes, where <strong>power dynamics affect participation<\/strong>.<\/li>\n<li><strong> Importance of Public Engagement:<\/strong> Citizen involvement <strong>affirms dignity, counters epistemic injustice, and strengthens democracy<\/strong>. It ensures that people influence health decisions affecting them.<\/li>\n<li><strong> Benefits for Service Delivery: <\/strong>Engaging communities <strong>improves accountability<\/strong>, reduces elite dominance, builds trust with providers, and <strong>enhances health outcomes<\/strong>.<\/li>\n<\/ol>\n<h2><strong>Institutional Mechanisms for Participation<\/strong><\/h2>\n<ol>\n<li><strong> National Rural Health Mission Platforms: <\/strong>The <strong>NHM (2005)<\/strong> created Village Health Sanitation and Nutrition Committees (VHSNCs) and Rogi Kalyan Samitis for inclusive participation, especially of <strong>women and marginalised groups<\/strong>, with <strong>untied funds<\/strong> for local initiatives.<\/li>\n<li><strong> Urban Participation Structures:<\/strong> Urban equivalents include <strong>Mahila Arogya Samitis<\/strong>, Ward Committees, and NGO-led bodies.<\/li>\n<li><strong> Challenges in Implementation:<\/strong> In some areas, <strong>committees are absent<\/strong>; in others, they face <strong>unclear roles, rare meetings, underused funds, poor coordination<\/strong>, and <strong>social hierarchies<\/strong>.<\/li>\n<\/ol>\n<h2><strong>Barriers to Effective Engagement<\/strong><\/h2>\n<ol>\n<li><strong> Perception of Citizens as Passive:<\/strong> Policymakers often see people as <strong>recipients, not co-creators<\/strong>. Performance is judged by <strong>target numbers<\/strong>, ignoring ground realities.<\/li>\n<li><strong> Language and Framing Issues:<\/strong> Terms like <strong>\u201c<\/strong><strong>beneficiaries<\/strong><strong>\u201d<\/strong> reduce citizens to <strong>objects of intervention<\/strong> rather than <strong>rights-holders<\/strong>.<\/li>\n<li><strong> Medicalised and Hierarchical Governance:<\/strong> Leadership is <strong>dominated by doctors<\/strong> trained in biomedical models. Promotions are often based on <strong>seniority<\/strong>, not public health expertise, creating a <strong>disconnect from community needs<\/strong>.<\/li>\n<li><strong> Resistance to Participation: <\/strong>Concerns include <strong>extra workload, accountability pressure, dominance by medical or capitalist interests<\/strong>, and <strong>unequal influence<\/strong> in governance spaces.<\/li>\n<\/ol>\n<h2><strong>Alternative Avenues for Citizen Voice<\/strong><\/h2>\n<ol>\n<li><strong>Non-institutional Channels<\/strong><strong>:<\/strong> Where formal platforms fail, citizens turn to <strong>protests, media campaigns, and legal action<\/strong>.<\/li>\n<li><strong> Unmet Demand for Inclusion: <\/strong>These actions reflect a <strong>strong need for voice and accountability<\/strong>.<\/li>\n<\/ol>\n<h2><strong>Path to Meaningful Community Engagement<\/strong><\/h2>\n<ol>\n<li><strong>Mindset Transformation<\/strong><strong>: <\/strong>Participation should be valued <strong>as much as outcomes<\/strong>. People must be seen as <strong>partners, not instruments<\/strong>.<\/li>\n<li><strong> Empowering Communities:<\/strong> Spread <strong>health rights awareness<\/strong>, engage <strong>marginalised groups<\/strong>, build <strong>civic education early<\/strong>, and provide <strong>tools for decision-making<\/strong>.<\/li>\n<li><strong> Sensitising Health Actors:<\/strong> Avoid blaming poor awareness for low service use.
