At the same time, the risk factors for disease are steadily rising due to policy gaps &amp; systemic policy failures. The consumption of ultra-processed foods is driving an epidemic of non-communicable diseases, while unchecked air, water, and soil pollution, along with climate change are pushing millions into poverty. In this scenario, the role of public healthcare becomes even more important.<br \/>\nIn this article, we will look at what constitutes a public health and the public health sector in India. We will look at the challenges faced by the public health sector in India. Provisioning of curative care is the <span style=\"color: #ff0000;\">most critical<\/span> and <span style=\"color: #ff0000;\">controversial policy question<\/span> in public health.<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<p><strong>Levels of Public Health Sector in India:<\/strong><\/p>\n<table style=\"height: 360px; width: 100%; border-collapse: collapse; border-style: solid; background-color: #fce8e8; border-color: #000000;\">\n<tbody>\n<tr style=\"height: 300px;\">\n<td style=\"width: 13.6546%; height: 300px;\"><strong>Primary Healthcare<\/strong><\/td>\n<td style=\"width: 86.3454%; height: 300px;\">India&#8217;s primary health sector consists of sub-centers (SCs) and primary health centres (PHCs), which form the foundation of the public healthcare system.<br \/>\n<strong>Sub-centers:<\/strong>\u00a0These are the <span style=\"color: #ff0000;\">most peripheral units<\/span>, serving populations of 5,000 in plain areas and 3,000 in hilly\/tribal areas.<br \/>\n<strong>Primary Health Centers:<\/strong>\u00a0These are the first point of contact with <span style=\"color: #ff0000;\">a qualified doctor<\/span>, serving <span style=\"color: #ff0000;\">populations of 20,000-30,000<\/span>. Each PHC is expected to have <span style=\"color: #ff0000;\">4\u20136 beds<\/span> and provide promotive, preventive, curative and rehabilitative care.<br \/>\nThe poor and the vulnerable rely on primary healthcare institutions of the public sector for primary-level care. It is the most affordable public health service and is closer to their places of residence.<\/td>\n<\/tr>\n<tr style=\"height: 30px;\">\n<td style=\"width: 13.6546%; height: 30px;\"><strong>Secondary Healthcare\u00a0<\/strong><\/td>\n<td style=\"width: 86.3454%; height: 30px;\">The secondary health sector in India consists of Community Health Centers (CHCs), which serve as referral units for PHCs.<br \/>\n<strong>CHCs<\/strong> are <span style=\"color: #ff0000;\">30-bed hospitals providing specialist care<\/span> in medicine, surgery, obstetrics &amp; gynaecology, and paediatrics. Efforts have been undertaken to follow the principles of primary health by strengthening the primary healthcare institutions under these missions. Inadequate Access to Healthcare:<\/strong>\u00a0Inadequate access to basic healthcare services remains a critical public health issue, particularly in rural and underserved areas. <strong>For ex-<\/strong> <span style=\"color: #ff0000;\">Shortage of health Infrastructure<\/span> and <span style=\"color: #ff0000;\">health professionals <span style=\"color: #333333;\">(shortage of around 600,000 doctors).<\/span><\/span><\/p>\n<p><strong>2. Myopic focus on short-term results:<\/strong>\u00a0There has been focus on promotion of initiatives that promise immediate results, such as <span style=\"color: #ff0000;\">opening new hospitals<\/span>, <span style=\"color: #ff0000;\">subsidised treatments<\/span>, and <span style=\"color: #ff0000;\">populist health policies<\/span>. The <span style=\"color: #ff0000;\">neglect<\/span> of the <span style=\"color: #ff0000;\">holistic development of primary and secondary healthcare capabilities<\/span> has led to deterioration of public health sector in India.<\/p>\n<p><strong>3. Low Healthcare expenditure by the Govt: <\/strong><\/p>\n<ul>\n<li>The <span style=\"color: #ff0000;\">government<\/span> (Centre and states put together) spends about <span style=\"color: #ff0000;\">1.9% of the GDP<\/span>. This is extremely low when compared to other government health expenditure in countries like <span style=\"color: #ff0000;\">China<\/span> (<span style=\"color: #ff0000;\">3%<\/span>), <span style=\"color: #ff0000;\">Thailand<\/span> (<span style=\"color: #ff0000;\">2.7%<\/span>), <span style=\"color: #ff0000;\">Vietnam<\/span> (<span style=\"color: #ff0000;\">2.7%<\/span>) and <span style=\"color: #ff0000;\">Sri Lanka<\/span> (<span style=\"color: #ff0000;\">1.4%<\/span>). Bhutan&#8217;s per capita spending on health was 2.5 times that of India, that of Sri Lanka was 3 times &amp; that of other BRICS nations was almost 14 times that of India in 2021.<\/li>\n<li>The National Health Policy (2017) had committed to increase the health expenditure by govt to 2.5% of GDP by 2025 &#8211; but this basic goal is nowhere near realization.<\/li>\n<li>During the COVID period, public spending on health had increased, but much of the rise attributed to the States rather than the Union govt. In fact, while the States have sustained such increase in post-COVID period (increased from 0.67% in 2017-18 to 1.1% in 2025-26), the Union govt&#8217;s spending on health, which increased moderately during the pandemic (from 0.32% of GDP in 2017-18 to 0.37% in 2020-21), has decreased to 0.29% of GDP in 2025-26.\n<p><figure id=\"attachment_354989\" aria-describedby=\"caption-attachment-354989\" style=\"width: 547px\" class=\"wp-caption aligncenter\"><img data-recalc-dims=\"1\" loading=\"lazy\" decoding=\"async\" class=\"wp-image-354989 size-full\" src=\"https:\/\/i0.wp.com\/forumias.com\/blog\/wp-content\/uploads\/2026\/01\/Screenshot-2026-01-30-150854.png?resize=547%2C520&#038;ssl=1\" alt=\"Public health \" width=\"547\" height=\"520\" srcset=\"https:\/\/i0.wp.com\/forumias.com\/blog\/wp-content\/uploads\/2026\/01\/Screenshot-2026-01-30-150854.png?w=547&amp;ssl=1 547w, https:\/\/i0.wp.com\/forumias.com\/blog\/wp-content\/uploads\/2026\/01\/Screenshot-2026-01-30-150854.png?resize=300%2C285&amp;ssl=1 300w\" sizes=\"auto, (max-width: 547px) 100vw, 547px\" \/><figcaption id=\"caption-attachment-354989\" class=\"wp-caption-text\">Source: The Hindu<\/figcaption><\/figure><\/li>\n<li>Health &amp; Education Cess, which was introduced in 2018-19 to expand the existing govt spending on health, has not been used to increase the health budget, but instead is being used to supplement the tax resources.<\/li>\n<li>Almost 75% of the Union govt&#8217;s spending on health was used to be transferred to the States in 2014-15 for various Centrally Sponsored Schemes like National Health Mission. But this share has consistently declined to reach just 43% in 2024-25 &#8211; which is completely insufficient to maintain basic health services.<\/li>\n<\/ul>\n<p><strong>4. Lack of adequate emphasis on critical areas:<\/strong>\u00a0There has been lack of adequate attention to critical areas such as <span style=\"color: #ff0000;\">sanitation<\/span>, <span style=\"color: #ff0000;\">disease surveillance<\/span>, and <span style=\"color: #ff0000;\">public health education<\/span>, which are important to maintain population health and prevent disease outbreaks. <strong>For ex-<\/strong> <span style=\"color: #ff0000;\">Lack of long-term strategies<\/span> like <span style=\"color: #ff0000;\">understanding vector control<\/span> or <span style=\"color: #ff0000;\">developing effective vaccines<\/span> in case of dengue.<\/p>\n<p><strong>5. Profit-Driven Pharma Sector:<\/strong>\u00a0The pharmaceutical industry\u2019s profit-driven nature <span style=\"color: #ff0000;\">often sidelines areas such as public health<\/span>. There has been a loss of trust towards healthcare providers (private sector due to commercial interests). <strong>For ex- <\/strong><span style=\"color: #ff0000;\">Medical marginalisation of TB patients in India<\/span>.<\/p>\n<p><strong>6. Lack of a Comprehensive Approach:<\/strong>\u00a0India\u2019s current public health approach is <span style=\"color: #ff0000;\">physician-centric<\/span>, without <span style=\"color: #ff0000;\">adequate focus on expertise from various fields<\/span> such as environmental science, sociology, urban planning, and economics.<\/p>\n<p><strong>7. Lack of Preventive Care:<\/strong>\u00a0Preventive healthcare is undervalued in India, despite its importance in reducing disease incidence and healthcare costs. This is especially significant as India faces a &#8216;<span style=\"color: #ff0000;\">triple burden<\/span>&#8216; of disease, comprising <span style=\"color: #ff0000;\">communicable diseases<\/span> (like tuberculosis and malaria), <span style=\"color: #ff0000;\">non-communicable diseases<\/span> (such as diabetes and heart disease), and <span style=\"color: #ff0000;\">emerging infectious diseases<\/span>.<\/p>\n<h2><span style=\"color: #000000;\"><strong><a id=\"toc4\"><\/a><span style=\"text-decoration: underline;\">What are the advantages of robust public health sector in India?<\/span><\/strong><\/span><\/h2>\n<ol>\n<li><strong>Improved the Access to Healthcare:<\/strong>\u00a0The Lancet in its latest study ranked <span style=\"color: #ff0000;\">India at 145th among 195 countries in terms of quality and accessibility of healthcare<\/span>, behind its neighbours China, Bangladesh, Sri Lanka and Bhutan. Thus, there is a need to improve the public health sector by making it more accessible by <span style=\"color: #ff0000;\">improving the quality of medical professionals<\/span> and <span style=\"color: #ff0000;\">infrastructure<\/span>.<\/li>\n<li><strong>Improved Health Outcomes: <\/strong>Robust public healthcare leads to early detection and treatment of diseases, resulting in improved health outcomes and a decrease in the burden of illness. <strong>For ex-<\/strong> <span style=\"color: #ff0000;\">Early detection and treatment of Non-Communicable diseases like Cardiovascular Diseases<\/span>.<\/li>\n<li><strong>Reduction of Financial Burden:<\/strong>\u00a0Improved public healthcare services can <span style=\"color: #ff0000;\">alleviate the financial burden<\/span> and <span style=\"color: #ff0000;\">improve household financial stability<\/span> by reducing the high out of pocket expenditure. <strong>For ex-<\/strong> According to the WHO, <span style=\"color: #ff0000;\">55 million people fall into poverty or deeper poverty every year<\/span> due to catastrophic expenditures on health.<\/li>\n<li><strong>Social Justice:<\/strong>\u00a0Universal healthcare and publicly funded health system provide timely, effective and free care, irrespective of social class. This in turn promotes social justice and <span style=\"color: #ff0000;\">fulfillment of DPSP principles<\/span>.<\/li>\n<\/ol>\n<h2><span style=\"text-decoration: underline;\"><span style=\"color: #000000; text-decoration: underline;\"><strong><a id=\"toc5\"><\/a>What Should be the Way Forward?<\/strong><\/span><\/span><\/h2>\n<ol>\n<li><strong>Increase Public Health Expenditure:\u00a0<\/strong>Raise public health spending from the current ~1.9-2% of GDP to at least 2.5-3%, as recommended by the National Health Policy 2017.<\/li>\n<li><strong>Transform Ayushman Arogya Mandirs (formerly HWCs):<\/strong>\u00a0Move beyond infrastructure to ensure they are\u00a0<strong>functional, well-staffed, and stocked.<\/strong> Empower them as the first point of comprehensive care.<\/li>\n<li><strong>Robust Human Resources:<\/strong> Address the massive shortage of doctors, nurses, and especially mid-level providers (Community Health Officers). Decentralize recruitment, improve working conditions, and create clear career pathways. Create a dedicated &#8220;All India Service&#8221; for public health (similar to the IAS) to manage health administration and policy at the state and district levels.<\/li>\n<li><strong>Leverage Telemedicine and AI:<\/strong> Use tele-consultation to bridge rural-urban gaps.
