In March 2026, the government informed Parliament that <strong>43 new medical colleges<\/strong>, <strong>11,682 MBBS seats<\/strong>, and <strong>8,967 postgraduate seats<\/strong> were approved for 2025-26. High vacancy rates in CHCs- <\/strong>According to the <strong>Health Dynamics of India 2022-23 report<\/strong>, Community Health Centres (CHCs) face a massive <strong>79.9% vacancy rate<\/strong>, with only <strong>4,413 specialists available against a requirement of 21,964 specialists<\/strong>. This highlights the severe shortage of doctors in rural healthcare.<\/p>\n<p><strong>2. Overburdened district hospitals- <\/strong>Due to poorly functioning CHCs, patients are often forced to travel long distances to district hospitals and medical colleges for basic specialist care, increasing pressure on tertiary healthcare institutions.<\/p>\n<p><strong>3. Infrastructure without operational readiness- <\/strong>Many newly built hospitals and health institutions lack essential medicines, diagnostic facilities, ambulance services, and adequate medical staff. Infrastructure creation alone cannot ensure better healthcare delivery.<\/p>\n<p><strong>4. Rising privatisation of medical education- <\/strong>Out of the <strong>43 newly approved medical colleges<\/strong>, <strong>27 belong to the private sector<\/strong>, which raises concerns about affordability and weakens public healthcare accountability.<\/p>\n<p><strong>5. Poor working conditions in remote areas- <\/strong>Doctors often avoid rural postings due to inadequate housing, lack of quality schooling for children, poor medical equipment, and limited professional support in remote areas.<\/p>\n<p><strong>6. Weak workforce planning- <\/strong>India produces thousands of specialists every year, yet vacancies in public health institutions remain unfilled due to poor coordination between medical education and healthcare workforce planning.<\/p>\n<p><strong>Way Forward<\/strong><\/p>\n<p><strong>1. Shift focus from infrastructure to health outcomes- <\/strong>Healthcare investments should move beyond constructing buildings and focus on actual service delivery. Availability of medicines, diagnostics, emergency care, and trained staff should become key indicators of success.<\/p>\n<p><strong>2. Provide incentives for rural service- <\/strong>Doctors serving in difficult areas should receive higher salaries, better housing facilities, education support for children, and faster career progression. States like Chhattisgarh have already experimented with such models.<\/p>\n<p><strong>3. Link postgraduate seats with mandatory service- <\/strong>Government-funded postgraduate medical seats can be linked with compulsory service in underserved areas to ensure public investment strengthens rural healthcare delivery.<\/p>\n<p><strong>4. Strengthen district-level healthcare systems- <\/strong>Instead of creating numerous poorly functioning CHCs, the focus should be on developing a few fully equipped and well-staffed healthcare centres in each district. Quality should take precedence over quantity.<\/p>\n<p><strong>5. Improve healthcare governance- <\/strong>Faster recruitment, better Centre-State coordination, transparent transfers, and stronger accountability mechanisms are necessary to improve public health administration.<\/p>\n<p><strong>6. Promote team-based healthcare delivery- <\/strong>Posting complete teams of specialists, nurses, and support staff together can improve efficiency, reduce stress on doctors, and enhance public trust in government hospitals.<\/p>\n<h2><strong>Conclusion<\/strong><\/h2>\n<p>India\u2019s health system requires structural reforms that go beyond physical infrastructure.
