A recent ICMR study across <strong>20 tertiary-care hospitals<\/strong>, covering <strong>1,59,336 hospitalised patients between April 2022 and April 2025<\/strong>, highlights the serious mortality and economic burden of carbapenem-resistant Gram-negative infections.<\/p>\n<h2 class=\"yellow-h2-box\"><strong>Why is the situation alarming?<\/strong><\/h2>\n<ol>\n<li><strong>Higher mortality:<\/strong> Carbapenem-resistant infections were associated with a <strong>16\u201343% higher relative risk of death<\/strong> across <em>E. coli, Klebsiella pneumoniae, Acinetobacter baumannii<\/em> and <em>Pseudomonas aeruginosa<\/em>.<\/li>\n<li><strong>High mortality in bloodstream infections:<\/strong> Mortality among resistant bloodstream infections ranged from around <strong>39% in resistant E. coli to 51% in resistant A. baumannii<\/strong>.<\/li>\n<li><strong>High resistance in tertiary hospitals:<\/strong> More than <strong>61% of Gram-negative infections<\/strong> in the study were carbapenem-resistant, although this figure should not be treated as representative of the general community because these hospitals largely handle referred and critically ill patients.<\/li>\n<li><strong>Higher treatment costs:<\/strong> Antibiotic costs for resistant infections were <strong>1.1\u20132 times higher<\/strong>, while the actual economic burden is likely to be much greater after including ICU care, diagnostics, procedures, supportive care and hospital charges.<\/li>\n<\/ol>\n<h2 class=\"yellow-h2-box\"><strong>Factors driving AMR beyond antibiotic overuse<\/strong><\/h2>\n<ol>\n<li><strong>Healthcare-associated transmission:<\/strong> More than <strong>85% of bloodstream infections<\/strong> in the study were classified as healthcare-associated, highlighting the role of transmission within hospitals.<\/li>\n<li><strong>Gaps in infection prevention:<\/strong> Inadequate hand hygiene, environmental cleaning and surgical infection prevention can facilitate the spread of resistant organisms.<\/li>\n<li><strong>Invasive medical devices:<\/strong> Catheters and other invasive devices increase infection risk, particularly when they are not removed at the earliest appropriate opportunity.<\/li>\n<li><strong>Recent surgery and critical illness:<\/strong> Surgical procedures, severe illness and multiple comorbidities can increase vulnerability to resistant infections.<\/li>\n<li><strong>Delayed diagnosis:<\/strong> Late recognition of infection delays appropriate treatment and allows patients to deteriorate.<\/li>\n<li><strong>Inadequate or delayed therapy:<\/strong> Resistance may not be identified quickly enough, leaving clinicians with fewer effective treatment options.<\/li>\n<li><strong>Antibiotic selection pressure:<\/strong> Excessive or inappropriate antibiotic exposure promotes the emergence and spread of resistant bacteria.<\/li>\n<\/ol>\n<h2 class=\"yellow-h2-box\"><strong>Way forward<\/strong><\/h2>\n<ol>\n<li><strong>Strengthen infection prevention and control:<\/strong> Hospitals must prioritise hand hygiene, environmental cleaning, device-associated infection prevention, surgical infection prevention and surveillance of healthcare-associated infections.<\/li>\n<li><strong>Improve diagnostics:<\/strong> Faster and more accurate diagnostics should help distinguish bacterial from non-bacterial illness and identify resistance at an early stage.<\/li>\n<li><strong>Ensure antimicrobial stewardship:<\/strong> Antibiotics should be used at the <strong>narrowest effective spectrum and for the shortest appropriate duration<\/strong>, rather than routinely escalating to stronger drugs.<\/li>\n<li><strong>Integrate AMR surveillance:<\/strong> India should move beyond isolate-based surveillance towards <strong>patient-level surveillance linking laboratory results with treatment and mortality outcomes<\/strong>.<\/li>\n<li><strong>Ensure responsible access to newer antimicrobials:<\/strong> Effective newer drugs should remain available for patients who genuinely need them while being protected through appropriate stewardship.<\/li>\n<li><strong>Use evidence cautiously:<\/strong> Drugs such as <strong>ceftazidime-avibactam<\/strong> showed beneficial associations in some resistant <em>E. coli<\/em> and <em>Klebsiella<\/em> infections, but observational evidence cannot establish universal superiority. Treatment should therefore be guided by diagnostics and resistance mechanisms.<\/li>\n<\/ol>\n<p><strong>Conclusion:<\/strong> <strong>AMR is not merely an antibiotic-prescribing problem; it is also an infection-prevention, diagnosis, transmission and healthcare-system problem.<\/strong> Antibiotics cannot compensate for failures in infection prevention. Therefore, India needs an integrated strategy combining <strong>prevention, early diagnosis, surveillance, antimicrobial stewardship and responsible access to effective newer treatments<\/strong>.<\/p>\n<p><strong>Question: <\/strong>Antimicrobial resistance (AMR) cannot be addressed merely by controlling antibiotic overuse.
