In 2011-12, more than 55 million Indians<\/strong> were pushed into poverty because of rising expenditure on healthcare. Government of India sue to rising healthcare challenges launched Ayushman Bharat: National Health Protection Scheme (AB-NHPS) which has a defined benefit cover of\u00a0\u20b9\u00a05 lakh per family per year covering over 10 crore families. Pegged to be world\u2019s largest healthcare scheme it has some issues and challenges.<\/p>\n\n\n\n

Various issues and challenges<\/strong><\/h3>\n\n\n\n
  1. A state subject-<\/strong> <\/strong>Under the 7th schedule of\nthe Indian Constitution, health is a state subject. Apart from central\ninstitutions such as the All India Institute of Medical Sciences, a major chunk\nof the hospitals\/ medical centres is state-owned and -operated. A nationwide\nscheme of health insurance to supply healthcare facilities at the state level\nleads to a dilution of the state responsibility in the provisioning of the\nsame. Further it require centre state on equal footing.<\/li>
  2. Not a long term solution<\/strong>– Conferring responsibility on private insurance could not be long term\nsolution. As public private partnerships in past have been criticised for\nvarious issues, above can also lead to issues in the implementation of Ayushman\nBharat. Further in rural areas private doctors are few as doctors are reluctant\nto work in rural areas. Mostly quacks or family doctor serve, so it can be an\nissue.<\/li>
  3. Infrastructure<\/strong>– Shortages in services such as infrastructure, Human Resources, quality\nof doctors etc have always been an issue. Many villages still lack roads,\nnearby hospitals, proper staff etc. Thus implementation of the scheme would be\na huge task.<\/li>
  4. Monetary-<\/strong> <\/strong>It requires huge amount of funding\nand spending. States have to mobilise funds which will not be an easy task\nbeing many states are under debt, further borrowing may put balance sheet under\nstress. Further there is no uniform pricing system for other health\ninterventions like medicines and diagnostics. <\/li>
  5. Not Universal-<\/strong> <\/strong>Developed nation has universal health\ncoverage, while scheme doesn\u2019t cover all sections of society, so many may\nremain excluded. It cover only 40% <\/em><\/strong>of Indian population. <\/li><\/ol>\n\n\n\n

    How it can do wonders?<\/strong><\/h3>\n\n\n\n
    1. Better Reach-<\/strong> <\/strong>Establishment of Ayushman Bharat National Health Protection\nMission Agency at National Level and State Health Agency will ensure\nimplementation of Scheme in a targeted manner ensuring effective implementation.\nThe States and UTs can implement scheme through various means like insurance\ncompany or Directly through Trust\/Society. This would increase ambit of the\nScheme at Ground levels.<\/li>
    2. Creation of healthcare Infrastructure<\/strong>– <\/strong>A strong Network of  the 1,50,000 Health and Wellness Centres being\ndeveloped will strengthen the national focus on preventive healthcare.<\/li>
    3. Targeted coverage-<\/strong> <\/strong>It will Cover more than 10 Crore Poor and Vulnerable Families\nof the Society. <\/li>
    4. Penetration and increased\naccess-<\/strong> <\/strong>Participation\nof private sector will not only ensure more doctors but also near home\navailability of health facilities. It will increase the availability near Home\nin Rural Areas through trained professionals. It will also increase access to Vulnerable\nSections of the Society to Healthcare. Further The scheme is portable in nature\nthus will also benefit the migrated population in unorganised sector which are\noften excluded in government schemes.      <\/li>
    5. Affordability-<\/strong> <\/strong>Another impact of the PMJAY\nwill be rationalisation of the cost of care in the private sector. <\/li>
    6. Employment-<\/strong> <\/strong>It will generate Employment\nand  will create lakhs of jobs for\nprofessionals and non-professionals \u2014 especially women. It will give a boost to\nthe health technology industry.<\/li>
    7. Better Healthcare System-<\/strong> It will be an enabler of\nquality, affordability and accountability in the health system. The empanelled\nhospitals have been tasked to follow the treatment guidelines. Patient outcomes\nwill be monitored.<\/li><\/ol>\n\n\n\n

      Government data suggests that about 63% of the\npeople<\/strong> have to pay for their own healthcare and hospitalisation expenses as\nthey aren\u2019t covered under any health protection scheme. Hence, the Ayushman Bharat\nscheme is well-intentioned. However, its implementation in the current form need\nextra efforts and can solve nation\u2019s health issues, which require state-centre efforts\nand intent.<\/em><\/p>\n","protected":false},"excerpt":{"rendered":"
