{"id":81788,"date":"2021-02-12T12:01:34","date_gmt":"2021-02-12T06:31:34","guid":{"rendered":"https:\/\/blog.forumias.com\/?p=81788"},"modified":"2021-02-12T17:15:12","modified_gmt":"2021-02-12T11:45:12","slug":"budget-2020-21-didnt-address-the-core-issues-of-ailing-health-sector","status":"publish","type":"post","link":"https:\/\/forumias.com\/blog\/budget-2020-21-didnt-address-the-core-issues-of-ailing-health-sector\/","title":{"rendered":"Budget 2020-21 didn\u2019t address the core issues of ailing health sector"},"content":{"rendered":"<p><strong>Source: <a href=\"https:\/\/indianexpress.com\/article\/opinion\/columns\/budget-health-sector-ayushman-bharat-covid-7181738\/\">Indian Express<\/a><\/strong><\/p>\n<p><strong>Gs2:<\/strong>\u00a0<strong>issues Relating to Development and Management of Social Sector\/Services relating to Health.<\/strong><\/p>\n<p><strong>Synopsis: <\/strong>Four important issues ailing the health sector did not find a place in the budget.<\/p>\n<p><strong>Background<\/strong><\/p>\n<ol>\n<li>The budget allocation for the health sector in 2021-22 was (<strong>24 lakh crore)<\/strong> 137 % more than the previous year (69,000 crores).<\/li>\n<li>However, the actual share of Health and Family Welfare was only a third of the total allocation of Rs. <strong>24 lakh crore<\/strong>.<\/li>\n<li>This is because, the increase in budget for \u2018Health wellbeing\u2019 also included funds for drinking water, sanitation, nutrition, AYUSH, health research, vaccination as well as grants assigned by the <strong>15th Finance Commission<\/strong>.<\/li>\n<li>Also, a new centrally-sponsored scheme <a href=\"https:\/\/forumias.com\/blog\/pm-aatmanirbhar-swasthya-bharat-yojana\/\" target=\"_blank\" rel=\"noopener noreferrer\">PM<\/a><strong> Aatmanirbhar Swasthya Bharat Yojana was announced as a part of Budget <\/strong>2021-22.<\/li>\n<li>Even in this case, the funds allocated to this new scheme will be materialized only when the programmes are implemented.<\/li>\n<\/ol>\n<p><strong>What are the critical issues that have not been addressed by the Budget 2021-22 in the health sector?<\/strong><\/p>\n<p>Despite the above measures introduced in the Budget 2021-22, it has failed to address the four critical issues ailing the health sector. They are<\/p>\n<ol>\n<li><strong>First,<\/strong> the Budget has failed to find a solution to bring down the costs<strong> of outpatient treatment. <\/strong>\n<ul>\n<li><strong>Under Aayushman Bharat, <\/strong>50 crores of eligible poor people are entitled to cashless hospitalisation.\u00a0However, the cost of drugs, the high cost of high-end diagnostics leads to <strong>high <\/strong><strong>out<\/strong><strong>-of-pocket expenditure <\/strong>before a patient needs hospitalisation.<\/li>\n<li>This ultimately reduces the number of people using affordable health care services. It defeats the very purpose of <strong>Aayushman Bharat.<\/strong><\/li>\n<\/ul>\n<\/li>\n<li><strong>Second,<\/strong> Insurance coverage for the middle class is not adequate.\u00a0Under the Ayushman Bharat health Programme, only 50-crore poor people are covered.\n<ul>\n<li>Whereas another \u00a010 per cent organised sector employees are covered by different government or employer generated medical coverage schemes.<\/li>\n<li>Still, around 30 crore Middle-Class Indians are without any state-supported medical insurance.<\/li>\n<\/ul>\n<\/li>\n<li><strong>Third,<\/strong> the Budget has failed to give effect to the 2017 <strong>National Health Policy recommendation<\/strong>. The Policy recommended for <strong>grading of clinical establishments<\/strong> to protect consumers rights.\u00a0The states are not implementing the model Clinical Establishments Act 2010.<\/li>\n<li><strong>Fourth,<\/strong> the need for a separate <strong>Empowered Medical Tribunal as <\/strong>recommended by 2017 <strong>National Health Policy, <\/strong>is\u00a0not been addressed<strong>.<\/strong> The complexities of medical negligence or malpractice cannot be addressed by The Consumer Protection Act 1986. Thus, a separate health regulator on the lines of the State and Central Regulatory Commissions is critical for the health sector.<\/li>\n<\/ol>\n<p><strong>What needs to be done?<\/strong><\/p>\n<ul>\n<li>A viable solution is required to reduce out of pocket expenditure in outpatient care services. This will make public health services affordable.<\/li>\n<li>Some international models can be analysed for health care funding:\n<ul>\n<li><strong>In Germany and France<\/strong>, health care is funded through mandatory contributions of both employer and employees.<\/li>\n<li><strong>The Canadian model:<\/strong> The Canada Health Act of 1984 makes federal cost-sharing a government responsibility. Healthcare entitlements in Canada are also portable across the country.<\/li>\n<\/ul>\n<\/li>\n<li>The <strong>Employees State Insurance Corporation<\/strong> scheme created in 1948 could also be modified and used to cover white-collar employees in the unorganized sector.<\/li>\n<li>Public health and hospitals can be brought on the concurrent list of the Constitution as recommended by the 15th Finance Commission.
