Yet concerns remain about empathy, safety, privacy, bias, and over-reliance. The key is how these tools are designed and governed—as a bridge to human care or as a substitute that delays timely clinical help. Recent data show over a million weekly suicide/self-harm chats with ChatGPT.
Argument in Favour of AI-based Tools for Mental Health
- Increased Accessibility and Availability:Chatbots and apps (e.g., Woebot, Wysa, and India’s Peakoo by Peak Mind) provide 24×7, on-demand support. This helps rural and underserved users reach help without travel or waitlists.
- Affordability: Many tools are low-cost or free, lowering barriers where therapy is expensive or hard to access regularly.
- Reduced Stigma:A private, judgment-free space can be the first step for users hesitant to approach a counsellor due to stigma.
- Support for Clinicians: AI can triage, summarise chats, track mood over time, flag risk patterns, and automate admin tasks, letting clinicians focus on complex cases.
- Early Detection and Monitoring: Algorithms can analyse text, speech cues, or phone-use patterns to spot early warning signs (depression, suicidality) for earlier intervention.
- Personalisation and Consistency: AI can provide consistent guidance and tailored prompts based on user patterns, helping users stick to routines.
- Psychoeducation at Scale : Apps can teach core skills (sleep hygiene, grounding, journaling) to large groups, easing clinician load.
- Multilingual and Accessibility Support : Bots can use multiple languages, simple text, and voice input, helping users with literacy barriers or disabilities.
Argument Against AI-based Tools for Mental Health
- Lack of Empathy and Human Connection: AI cannot replicate genuine empathy or the therapeutic alliance, which are central to effective therapy.

- Human-in-the-Loop and Escalation: Default handoffs to counsellors/psychiatrists for risk signals; real-time alerts to designated authorities.
- Clear scope and honest framing: Present AI as a tool, not a therapist. State what the tool can and cannot do before any interaction.
- Evidence and clinical oversight: Use validated screenings and clinically approved protocols. Review outcomes regularly with clinicians. Remove features that are inaccurate or cause harm.
- Privacy by design: Collect the minimum data needed. Pseudonymise user identity. Use plain-language consent that explains who can see data, for what purpose, and when escalation happens.
- Bias testing and inclusive design: Test models across languages, regions, genders, and marginalised groups. Involve diverse Indian users in design and red-team evaluations. Fix detected bias quickly.
- Crisis readiness: Build in crisis buttons, helpline routing, and location awareness. Enable immediate human takeover when there is self-harm, violence risk, or severe distress.
- Usage limits and referral thresholds: Cap session length and frequency. If a user returns often or signals worsen, mandate referral to a human counsellor and pause further bot chats.
- Ecosystem integration: Connect apps to campus counselling, peer groups, and family supports. Let AI handle admin and progress tracking so clinicians can focus on complex care.
